Register with the practice
We are accepting new patients and have an open list. Anyone can register with the practice as long as they are in our practice area we currently cover Houghton, Parts of Hetton, Shiney Row, Bournmoor, Warden Law, Chilton Moor, Hazard Lane up to Four Lane Ends for exact areas please call and speak to one of our receptionists. You will then need to come into the practice and complete a GMS1 form, we will need to see proof of ID in the form of passport, birth certificate and utility bill/rent book with proof of current address etc.
You will then be required to complete a General Health Questionnaire which we will add to your records. It is important that you give us as much information as possible as it can take up to 2 weeks to receive your records depending on where you were last registered.

We offer same day appointments or up to 2 weeks in advance.
Appointments can be booked by telephone on (0191) 5842106 lines open at 08:30 Monday to Friday, for advance bookings please call between 10.00 am and 17:00
To book appointments on line you will need to visit our web site and register for this service you will then be issued with your unique log in details.
We always try to minimise the waiting time for you and, whilst we try to see you at the appointed time, some patients’ appointments may take longer than the time allocated, so please be patient if there is some delay. We will also try to give you an appointment with the doctor that you prefer, but it may not always be possible.
Get a blood test
When you are told by a doctor that you need a blood test you should make an appointment with either of our Healthcare Assistant. The doctor will put on your notes what tests are required.
If you are told that you need a fasting blood test the doctor will explain the reasons for this and what you need to do. You should tell the receptionist this so that they can arrange an early morning appointment for you.
Get test results
You can ring the surgery any time during normal surgery hours to obtain blood test results. Most tests are available after 5 days but some tests do take longer. The receptionist can only give you the message attached to the result as authorised by the doctor. They will not be able to discuss the results further. Alternately you can request online access from the surgery you will need to follow the instructions in from our website or contact the surgery for more information.
Results can only be given to the patient who has had the tests done and only with the doctor’s approval. If you have any queries about your results discuss them with the doctor either on the telephone or by making an appointment.
